55TH REUNION CELEBRATION

Those attending the reunion will receive the Directory when you

register/sign in on Friday/Saturday. A picture of the classmate’s group

picture is also included in the registration fee and will be mailed after

the reunion. 

If you want a copy of the otherschool’s group picture, the cost will be $8.00

(a little less expensive because it will be sent in the same envelope with the

other picture). Please add the $8.00 to your registration fee. If you are not

able to attend but would like to order the OHS/PHS Directory, it will be $10.00

and will be sent after the reunion.  We are also offering a copy of the 8 x 10

group picture of each class for $10.00 apiece. The cost for both pictures will

be $18.00. Shipping is included in prices.

Registration:  Cost is $55.00 per person. This includes 1 group picture,

Directory, sandwiches, snacks, etc. on Friday night and a buffet dinner

on Saturday night. Dessert is provided on both nights. A cash bar will be

I'm available for both nights. We are cutting back on the planned activities this

time giving everyone more free time. There will be no partial or reduced

attendance fees. Your check should be made out to the OHS/PHS

Classes of 1961 Reunion and should be mailed along with this form

You will receive an email confirmation when your registration form and check

are received.  If you are paying for a spouse, friend, etc., please provide name

and connection__________________________. This reunion highlights the

OHS-PHS Classes of 1961’s 55th celebration but EVERYONE from all classes

and schools is welcome and will be registered as guests when their registration

fee is paid. We are taking donations to help with the reunion expenses if you

would like to make a donation.  I would like to make a donation of $_______

Hotel:  Please make your own hotel reservation. Registration and all

evening activities will be held at the MCM Elegante.  They have given us

special contracted prices:  $99.99 for a standard room (King or Double

Queen) with the same price for handicapped accessible rooms. Room prices do not

include taxes. THIS DISCOUNTED RATE IS ONLY AVAILABLE UNTIL FRIDAY,

SEPTEMBER 9, 2016 SO IF YOU ARE PLANNING TO COME, MAKE
